The challenge

Clontarf Hospital provides specialist rehabilitation services through multidisciplinary teams supporting patients with complex care needs.

Clinical teams captured important information about patient assessments, interventions and progress, but much of it was held in separate spreadsheets that were difficult to manage, combine or analyse consistently.

Connecting clinical information with the hospital's Patient Administration System was difficult, limiting a complete view of patient activity, clinical caseloads and outcomes. Reporting required significant manual effort and information could not easily be shared across services.

The hospital needed a secure, reliable application supporting structured clinical data capture across multiple disciplines, integrated with existing hospital data and available to teams operating around the clock.

02

Our approach

MedModus worked closely with clinical and operational teams to understand how information was collected, how services operated and what teams needed from their reporting.

Rather than replacing existing processes with a rigid, standardised product, the application was developed around the hospital's clinical workflows. Each area captures the information required by the relevant discipline while contributing to a consistent, hospital-wide data structure.

The design supports different clinical disciplines, connects clinical information with existing patient and admission data, reduces duplicate entry and reliance on standalone spreadsheets, provides secure role-appropriate access, automates the production and updating of reports, and remains flexible as requirements evolve — with reliable availability for teams working 24 hours a day, seven days a week.

The solution was introduced incrementally, allowing the hospital to expand it across additional services and adapt it as requirements changed.

03

The solution

MedModus developed a secure web application that brings clinical data capture and reporting together in a single environment, supporting Occupational Therapy, Physiotherapy, Speech and Language Therapy, Dietetics, Social Work, Psychology and Nursing.

Clinical teams select the relevant patient and record structured information through workflows designed around their specific assessment and service requirements. Existing patient and admission information is incorporated into the application, reducing the need to repeatedly enter core administrative details.

The captured information is combined with hospital data and made available through interactive reports, giving teams up-to-date visibility of activity, caseloads, assessment completion and patient outcomes without consolidating spreadsheets or preparing recurring reports manually.

Occupational Therapy shows how the application supports outcomes-based reporting. Therapists record structured assessments at key stages of the patient journey — baseline, admission and discharge — so changes in functional measures can be analysed over time, providing clearer evidence of patient progress and the impact of clinical interventions.

The application is securely hosted and managed by MedModus, providing a complete service covering access, data capture, integration, reporting and ongoing support.

04

The outcomes

The application has given Clontarf Hospital a consistent and scalable approach to managing clinical information across multiple disciplines.

It supports approximately 65 users across 12 clinical disciplines and services. MedModus continues to support and develop the application, which has been expanded since its initial implementation and is modified as requirements evolve — providing a sustainable clinical data platform that can support new workflows, reporting requirements and services without introducing further disconnected systems.

“MedModus has worked closely with our clinical and operational teams to develop a system that supports how our services work in practice. Just as importantly, they continue to provide a responsive and dependable support service, resolving issues quickly and adapting the application as our requirements evolve. This ongoing partnership gives us confidence that the system will continue to support our teams and develop alongside the hospital's needs.”

The application in use

MedModus clinical application showing structured Occupational Therapy FIM/FAM score capture at admission, goal and discharge, alongside a discharge submission list and monthly data quality check
Structured capture in Occupational Therapy: modified FIM scores recorded at admission, goal and discharge, with a live view of discharges still to be submitted and a monthly data quality check. Patient identifiers have been redacted.
Interactive reporting showing average FIM admission scores by month across functional measures and a radar chart comparing average admission, goal and discharge scores
Automated outcomes reporting built on the captured data: average admission scores by month across each functional measure, and admission, goal and discharge scores compared side by side to evidence patient progress.
